Orphan Patient Follow-up Clinic Project

Faced with the healthcare access crisis in Quebec, where many people lack a family doctor, the Orphan Patient Follow-up Clinic offers a sustainable and compassionate solution. Launched in 2024, it provides comprehensive care for 400 orphan patients through an interprofessional approach bringing together pharmacists, nurses, and support staff.
By redefining the role of the pharmacist, mobilizing every professional, and empowering each pharmacy technician (ATP) to reach their full potential, this initiative addresses a dual challenge: freeing up time for pharmacists so they can devote more attention to clinical activities, preventing the decline in the health of orphaned patients, and improving access to primary care.

PRISM Project: A Revolution in Mental Health Care for People Experiencing Homelessness

Homelessness is currently a public health crisis in Quebec, affecting a growing number of people living with complex mental health and addiction issues. Between 2018 and 2022, the number of homeless individuals increased by 44%, with an estimated annual economic burden of over $700 million, and the trend continues to rise.
The PRISM program (Projet Réaffiliation Itinérance Santé Mentale), launched in 2017 at the CIUSSS du Centre-Ouest-de-l’Île-de-Montréal in partnership with the Mission Bon Accueil shelter, offers a humane and innovative response to this reality. It aims to support the most marginalized individuals by offering a model rooted in their living environments, rather than imposing the traditional hospital-centered paradigm.
