Since the recent success of the home hospitalization program created at the CIUSSS Centre-Ouest during the Hippocrates Award Gala, we have seen significant progress both in terms of recognition for our initiative and its implementation. Winning the Hippocrates Award marked a turning point, not only by highlighting our team\u2019s innovation and commitment but also by boosting our visibility across the province. As a result, eight new sites are ready to adopt this care model, demonstrating growing acceptance and interest within the healthcare network. Furthermore, within the CIUSSS Centre-Ouest itself, development has continued. More than 950 users<\/strong> have benefited from our program to date, and we offer over 30 different care pathways<\/strong> to patients.<\/p>\n The impact of our program on creating value in healthcare is tangible, as it allows patients to receive personalized care in the comfort of their own homes, which represents a significant improvement in their quality of life. We do not merely offer an innovative care model; we improve our patients\u2019 health, and this is why we are committed to healthcare.<\/p>\n By measuring value, we aim to demonstrate how we improve outcomes that matter to patients. We are pleased with our results. In fact, 100% of patients prefer the home hospitalization program<\/strong> to hospital care. Users\u2019 self-reported health status is rated at 80%. And 88% of caregivers recommend the program<\/strong>, underscoring the importance of our person-centered approach. Furthermore, to date, we have added the equivalent of 4,000 bed-days to hospital capacity<\/strong>. This represents a significant expansion of access for the population, leading to increased efficiency in the healthcare system.<\/p>\n Transferability is ongoing and essential! Just recently, the provincial community of practice joined our organization<\/strong> to discuss best practices in virtual care delivery. These same sites are now launching their own programs.<\/p>\n Our virtual care program has expanded locally, with several other initiatives rolled out across the organization, such as Hololens technology in our long-term care and home care facilities, and the launch of point-of-care testing, which provides results for viral and blood tests in 15 minutes. This has been very beneficial for our seniors in home care<\/strong>, sparing them multiple trips to the emergency department.<\/p>\n The hospital-at-home model has proven its ability to deliver high-quality care in a more humane and cost-effective manner by decentralizing care from hospitals to communities. The benefits include better allocation of hospital resources, greater patient satisfaction, and a contribution to the healthcare system\u2019s resilience in the face of current and future challenges.
