\nOrphan Patient Follow-up Clinic Project
\nTransformative Category<\/p>\n

Faced with the healthcare access crisis in Quebec, where many people lack a family doctor, the Orphan Patient Follow-up Clinic offers a sustainable and compassionate solution. Launched in 2024, it provides comprehensive care for 400 orphan patients through an interprofessional approach bringing together pharmacists, nurses, and support staff.<\/p>\n

By redefining the role of the pharmacist, mobilizing every professional, and empowering each pharmacy technician (ATP) to reach their full potential, this initiative addresses a dual challenge: freeing up time for pharmacists so they can devote more attention to clinical activities, preventing the decline in the health of orphaned patients, and improving access to primary care.<\/p>\n

\nPresented by the CIUSSS du Centre-Ouest-de-l\u2019\u00cele-de-Montr\u00e9al<\/p>\n

Homelessness is currently a public health crisis in Quebec, affecting a growing number of people living with complex mental health and addiction issues. Between 2018 and 2022, the number of homeless individuals increased by 44%, with an estimated annual economic burden of over $700 million, and the trend continues to rise.<\/p>\n

The PRISM program (Projet R\u00e9affiliation Itin\u00e9rance Sant\u00e9 Mentale), launched in 2017 at the CIUSSS du Centre-Ouest-de-l\u2019\u00cele-de-Montr\u00e9al in partnership with the Mission Bon Accueil shelter, offers a humane and innovative response to this reality. It aims to support the most marginalized individuals by offering a model rooted in their living environments, rather than imposing the traditional hospital-centered paradigm.<\/p>\n
